The Dream House Family for Keeps® – Transition Care Program provides temporary, family-centered home care for medically fragile children while their own family members and homes are prepared to care for them.
This facility has been retrofitted to meet the needs of any medically fragile child age two weeks to 19 years. Learn who we serve.
Foster, kinship, and adoptive medically fragile children may be considered for admission to the Lilburn Transition Care Home.
Dream House accepts referrals from various sources, including but not limited to, public and private agencies, physicians, hospitals, and nursing homes.
Up to six medically fragile children may temporarily reside in the home, at any given time, for individual periods 30 days up to one year. On average, children in our care stay 90 days until Dream House staff transition them into safe and loving homes uniquely prepared to help them thrive.
