An integral part of our transition care model provides access to a multidisciplinary team who develop recommendations based on home assessment. With your financial support, we help families (biological, foster and adoptive) obtain home modifications and durable medical equipment to provide for safe and accessible homes.
Throughout 2007, Dream House Family for Keeps™ Bridging the Gap Program served 83 families in Georgia caring for 103 medically fragile children.
This program has a waiting list. Your financial donation will allow us to meet these needs as they arise. Below are the most frequently requested items.
- $ 1,000 will fund an assistive device needed for a child's daily care (bath chair, feeder seat, specialized car seat)
- $ 3,500 will fund a specialized mattress or positioning device
- $ 5,000 will fund a handicap access ramp for entry into the home
- $ 5,000 will fund bathroom accessibility equipment
- $ 7,500 will fund an overhead ceiling lift
- $ 7,500 will fund a stairway chair lift
- $10,000 will fund a bathroom modification (renovation)
- $15,000 will fund a handicap lift to retrofit a van
- $40,000 will purchase a used handicap-accessible van with lift
