RN Certified Case Manager

• Qualifications: Current Georgia license as a Registered Nurse, Certification as a Case Manager (CCM); minimum 1 year as a case manager working in a health care setting; pediatrics experience; current CPR and First Aid trainer certifications; satisfactory criminal record/background and health screening result; current Georgia drivers license and automobile insurance
• Preferred Experience: 2 years working with medically fragile children
• Position: Full Time 32-40 hours/week including some nights and weekends
• Location: the Dream House Family for Keeps® Transition Care Home (a licensed Children’s Transition Care Center in Lilburn, GA)
