Family for Keeps®
Transition Care Home

Dream House provides a "half-way home" (step-down) facility with 24-hour nursing care and skills training to prepare each child and family for transition to their home, at the flagship Family for Keeps® Transition Care Home in the Atlanta, Georgia suburb of Lilburn, licensed as the first and only Children's Transition Care Center (CTCC) in the United States.

Dream House relies on private donations and community support to provide services to Georgia's medically fragile children.

About the Transition Care Home

How to Make a Referral 

The Dream House community of supporters built, furnished, equipped, and continues to help fund program operations and maintain the 8,400 s.f. Transition Care Home that is a statewide resource for Georgia's most vulnerable children.

Staffed by registered nurses and certified nursing assistants, this Home is a 24/7 "awake" facility.
