Impact and Outcomes
The Dream House organization set a goal to serve 20 medically fragile children in 2011 at the Family for Keeps® Transition Care Home, Georgia's only licensed Children's Transition Care Center (CTCC). 

Since the first of the year, 31 children have received the best in skilled nursing, transition and respite care services at Georgia's first hospital-to-home resource for these kids, crushing the annual goal with four months to go! Seven of these children represent the highest level of care as they are sustained by mechanical ventilation. Year to date, Dream House has provided 438 transition care days, and 204 respite care days.
