- Our Transition Care Home provides temporary, family-centered home care for medically fragile children while their own family members and homes are prepared for them. We recruit, prepare and train foster families and respite care providers.

For Agencies (Hospitals, Insurance, DFCS): Referral & Admission Guide - Our Education and Skills Training Curriculums (Basic Caregiver, Advanced Workshop Series, Professional eCourse, and Community Care) are interactive learning experiences that teach families, caregivers, social work and other non-medical professionals how to provide safe and effective care for medically fragile children at home.
- Our Bridging the Gap Program helps families (biological, adoptive or foster) acquire therapies, equipment, health care supplies and home modifications to provide safe and accessible homes to meet their children's specialized needs.
- Our Advocacy Program strives to heighten public awareness for the plight of medically fragile children by linking private and public sector initiatives to meet the specialized needs of this rapidly growing, vulnerable pediatric population.

NURTURE THE DREAM
Sophie is such a fighter and an inspiration to anyone who has met her.
She was born with a rare condition called Arthrogryposis. Her diagnosis was very bleak, yet, Sophie has proven all her doctors wrong. She cannot move her arms or legs, but crawls using her head to pull her, and her knees to scoot around. She rolls to anything she wants. She paints, draws and colors with her mouth and toes.
I cannot thank Dream House enough for the donation that allowed Sophie to get a mouth stick to help with her PT/OT.
